About the Role:We are seeking a dedicated and experienced Occupational Therapist to join the 0-25 Disabilities Team on an agency basis. This role offers the chance to make a significant impact in the lives of children, young people, and young adults with disabilities, helping them to achieve greater independence and improve their quality of life.

As an Occupational Therapist in this team, you will work closely with families, carers, and other professionals to provide comprehensive assessments and tailored interventions.

Key Responsibilities:

  • Conduct holistic assessments of children and young people with disabilities to identify their functional and developmental needs.
  • Develop and implement individualised care plans, focusing on promoting independence and enhancing participation in daily activities.
  • Provide specialist advice, equipment recommendations, and adaptations to improve accessibility at home, school, or in the community.
  • Work collaboratively with families, carers, and multi-agency partners to deliver person-centered interventions.
  • Maintain accurate and up-to-date case records in compliance with professional and statutory standards.
  • Participate in multi-disciplinary meetings and contribute to service planning and development.
  • Support transitions between services for young people moving into adulthood.

Essential Skills and Qualifications:

  • Degree or Diploma in Occupational Therapy.
  • Registration with the Health and Care Professions Council (HCPC).
  • Experience working with children and young people with disabilities and their families.
  • Knowledge of relevant legislation, including the Children and Families Act 2014 and the Care Act 2014.
  • Strong assessment, analytical, and decision-making skills.
  • Excellent communication and interpersonal skills, with the ability to work in a multi-agency context.
