An exciting opportunity has arisen for an Occupational Therapist to work within the community to support individuals in maintaining their independence, safety, and well-being.

Key Responsibilities:
  • Conduct holistic assessments, support planning, and reviews in line with the Care Act 2014.
  • Provide tailored advice and interventions, including equipment and home adaptations.
  • Work closely with multi-disciplinary teams, families, and partner agencies to improve outcomes.
  • Undertake safeguarding investigations and promote best practices in adult protection.
  • Promote independence and self-management while considering cost-effective solutions.
  • Maintain accurate records and ensure compliance with relevant legislation and policies.
Requirements:
  • Recognised Occupational Therapy qualification and current HCPC registration.
  • Experience working with vulnerable adults and knowledge of adult safeguarding principles.
  • Strong assessment and problem-solving skills, with the ability to work independently and within a team.
  • Excellent communication and interpersonal skills.
  • Ability to manage caseloads effectively and prioritise tasks.
  • Must have own transport to travel across various locations as required.
