Randstad are recruiting for a Qualified Social Worker to join Lancashire County Council's Home first reviews and bed base reviews following hospital discharge and hospital avoidance Team. This is a full time role, offering an amazing pay rate of £37 per hour.

Responsibilities:

• Complete scheduled and unscheduled reviews for older adults in residential or nursing home care across Lancashire.
• You will ensure that quality of life and provision of support is person centred and strengths based using a Three Conversations approach.
• You will work closely with the Assurance team, our colleagues in safeguarding and commissioning.
• You would also undertake section 21a appeals work and provide critical analysis of information and write reports to Court standard.

Requirements:

• Social Work England registered
• Enhanced DBS
• Social Work Degree or equivalent
