Job Title: Adults Social Worker - Community Mental Health Team

Location: Rhondda Cynon Taf
Salary: £34 ph

About the Role:

Randstad are working closely with RCT to recruit a dedicated and compassionate Adults Social Worker to join our dynamic Community Mental Health Team. This role offers an opportunity to make a real difference in the lives of adults facing mental health challenges. You will work collaboratively with service users, families, and multi-disciplinary teams to deliver tailored support and interventions aimed at promoting independence, recovery, and well-being.



Key Responsibilities:

  • Assessment and Support Planning: Conduct holistic assessments of service users with mental health needs, developing individualized care and support plans.
  • Risk Management: Identify and manage risks, ensuring safety and well-being of service users while balancing the principles of empowerment and autonomy.
  • Multi-disciplinary Collaboration: Work closely with healthcare professionals, housing services, and other agencies to ensure integrated, person-centered care.
  • Safeguarding: Recognize and respond to safeguarding concerns, taking action in accordance with local authority policies and procedures.
  • Advocacy and Empowerment: Support service users to access the services and resources they need, advocating on their behalf to promote inclusion and recovery.
  • Case Management: Maintain accurate and timely records, manage caseloads effectively, and ensure that care plans are regularly reviewed and updated.
  • Crisis Intervention: Provide immediate support and intervention in times of mental health crisis, ensuring appropriate services and care are in place.


About You:

  • Qualifications: A degree in Social Work (or equivalent) and registration with Social Work England.
  • Experience: Proven experience in adult social work, ideally within a mental health setting or community-based services.
