Are you a passionate and experienced Adults Social Worker looking for your next challenge? Merthyr Tydfil County Borough Council is seeking a dedicated Agency Adults Social Worker to join our Front Door Team. This team plays a vital role in ensuring swift and effective responses to the needs of vulnerable adults, providing initial assessments and triaging referrals.

• Provide timely, proportionate assessments for adults referred to the Front Door Team.
• Triage referrals and determine appropriate levels of intervention or support.
• Collaborate with other teams, services, and external agencies to ensure a holistic approach to service users.
• Work with individuals and families, assessing their needs and implementing care and support plans.
• Ensure safeguarding principles are applied consistently, with a focus on risk assessment and risk management.
• Maintain accurate case records and comply with statutory responsibilities.

• Qualified Social Worker with relevant registration (Social Care Wales or equivalent).
• Experience working in adults services, ideally in a front-door or intake setting.
• Strong assessment, communication, and risk management skills.
• Ability to manage a varied caseload and work in a fast-paced environment.
• A sound understanding of safeguarding procedures and legislation.
• Access to your own transport and a willingness to travel within the Merthyr Tydfil area if required.
