About the Role:

Randstad are working to recruit a dedicated Adults Social Worker to join our Long-Term Team, supporting adults with a variety of complex needs to live independently and safely within the community. As part of the team, you will manage long-term cases, developing and maintaining meaningful relationships with service users to ensure their care and support plans evolve with their changing needs.



Key Responsibilities:

  • Case Management: Manage a caseload of adults with long-term needs, developing and reviewing care and support plans to ensure they continue to meet the individual's needs.
  • Assessment and Care Planning: Conduct person-centered assessments and reassessments, developing support plans that promote independence and well-being.
  • Safeguarding: Identify and respond to safeguarding concerns, ensuring that appropriate protection measures are in place in accordance with local authority policies.
  • Building Relationships: Develop and maintain long-term relationships with service users, their families, and carers to ensure continuous, holistic support.
  • Multi-agency Collaboration: Work closely with health professionals, housing services, and other partner agencies to coordinate support and deliver comprehensive care.
  • Legal and Policy Compliance: Apply relevant legislation, including the Care Act, Mental Capacity Act, and Deprivation of Liberty Safeguards, to ensure appropriate service delivery.
  • Advocacy and Empowerment: Advocate for service users to ensure they can access the services and resources they need, promoting their independence and inclusion.
  • Record Keeping: Maintain accurate case records and ensure all documentation is up to date, reflecting changes in care plans and service user circumstances.


About You:

  • Qualifications: A degree in Social Work (or equivalent) and current registration with Social Work Wales
