Locum Pharmacist - Band 8a (GP Surgery, Hampshire)
Pay: £32 - £35 per hour
5 Days a Week | 3-6 Month Contract | Starting ASAP

We are looking for an experienced and highly motivated Locum Pharmacist to join a dynamic GP Surgery team in Hampshire. This is a fantastic opportunity for a skilled pharmacist to lead on medicines management and medicines optimisation for complex patient cohorts, including those with cardiovascular, frailty, pain, and mental health conditions.

Role Details:
  • Position: Locum Pharmacist – Band 8a
  • Pay: £32 - £35 per hour
  • Contract Length: 3-6 months
  • Hours: Full-time, 5 days a week
  • Start Date: ASAP
Key Responsibilities:
  • Lead medicines management for a designated cohort of patients, with a focus on high-complexity areas such as cardiovascular, frailty, pain, and mental health.
  • Provide expert advice on polypharmacy, offering alternatives and reducing medication wastage within the practice.
  • Manage transfer of care between settings, including medicines reconciliation and liaising with local pharmacies to ensure continuity of care.
  • Deliver medicines reviews for patients within your specialist area, as well as general reviews for complex patients.
  • Support the development of individualised pharmaceutical care plans, and provide guidance on improving patient outcomes.
  • Educate and train clinicians and prescription teams on managing complex prescribing problems.
  • Rationalise repeat prescription lists, reduce wastage and duplication, and promote repeat dispensing services.
  • Promote self-care, health promotion, and wellbeing advice where appropriate.
  • Develop and implement prescribing guidelines in collaboration with secondary care services and CCGs to align with local and national medicines optimisation initiatives.
  • Lead on the critical appraisal of published literature and interpretation of complex health data to inform clinical decisions.
  • Ensure compliance with medicines optimisation programmes at local and national levels.
Ideal Candidate:
  • Registered Pharmacist with Band 8a experience or equivalent.
  • Proven experience in primary care settings, particularly within GP surgeries.
  • Expertise in medicines management, polypharmacy, and medicines optimisation.
  • In-depth knowledge of specialist therapeutic areas (cardiovascular, frailty, pain, mental health).
  • Strong communication and interpersonal skills, with the ability to collaborate effectively with clinical teams and patients.
  • Ability to manage a varied workload and take initiative in developing patient-centred pharmaceutical care plans.
