We are looking for an experienced Care Coordinator to join our team and support the development of a bespoke Community Care/Support Service across Buckinghamshire.The role involves coordinating individualized care services for clients, enabling them to remain in their homes and communities.

What you will do:

  • Ensure compliance with policies, procedures, and regulations.
  • Conduct client assessments, reviews, and update care plans as needed.
  • Liaise with families, medical professionals, and stakeholders for comprehensive support.
  • Schedule carers, manage staff hours, and update recruitment files.
  • Conduct staff supervisions, spot checks, and appraisals.
  • Provide on-call cover and address punctuality or continuity issues.
  • Promote equality, diversity, and inclusion.
  • Deliver care in the community and transport staff if required.
  • Identify and implement service improvements.

What you will have:

  • At least 3 years of experience in providing care (required).
  • NVQ Level 3 Health and Social Care
  • A valid UK Driving Licence (required).
  • Excellent knowledge of the Buckinghamshire area.
  • Willingness to travel (up to 75%).
