Responsibilities:

  • Coordinate safe and timely discharges from hospital
  • Assess the needs of patients and their families
  • Develop and implement care plans
  • Liaise with health professionals and community services
  • Provide support and advice to patients and their families

Requirements:

  • Degree in Social Work
  • Registered with the Scottish Social Services Council (SSSC)
  • Experience in hospital discharge planning
  • Knowledge of relevant legislation and policies
  • Excellent communication and interpersonal skills
