Job Overview

We are seeking an experienced Band 6 Occupational Therapist to join our community team in Marlow and Thame. This role focuses on elderly care, conducting community visits, and delivering person-centered therapy to enhance patients' quality of life and independence.

Key Responsibilities
  • Manage a community-based caseload, focusing on elderly care and rehabilitation.

  • Conduct home assessments and implement interventions to improve safety and independence.

  • Collaborate with multidisciplinary teams to coordinate comprehensive care plans.

  • Provide advice and support to patients, families, and carers to achieve therapy goals.

Key Requirements
