Social Worker: Adult's Service

Permanent, hybrid role

Specific Accountabilities of the Role:

  • To provide a social work service for adults with disabilities, and their families and carers, in line with job description below.
  • To act as a social worker providing effective social work services to adults with disabilities and their families and carers, with the purpose of safeguarding them, promoting their welfare, wellbeing and independence, respecting their dignity and choice, and aiming to prevent, reduce or delay the need for care and support.
  • To discharge the authority’s responsibilities under the Care Act 2014 and other relevant legislation in respect of adults with disabilities.
  • To support service users, and their families and carers, ensuring those who lack capacity receive appropriate support and advocacy to express their views and wishes, and that their social care needs are fully assessed.
  • Undertake carers’ assessments, recognising and supporting carers in their role, while enabling them to maintain a life beyond their caring responsibility.
  • Undertake assessments, including Best Interest Assessments: o Carry out best interest assessments as set out in statute.
  • Document and evidence best interest assessments as required, within the statutorily prescribed time frames.
  • Where required, identify and nominate a representative for the relevant person.
  • Make recommendations for any proposed authorisations whether that authorisation is approved or declined.
  • Engage in yearly refresher/review training of best interest qualification.
  • Regularly update skills and knowledge through local and national networks.
