We are looking for a dedicated and compassionate Social Worker to join our dynamic team and make a meaningful difference.

Role Overview:

As a Social Worker in the Adults Localities Team, you will be responsible for assessing the needs of older adults and individuals with physical disabilities. You will work collaboratively with service users, their families, and other professionals to develop and implement care plans that promote independence, well-being, and quality of life.

Key Responsibilities:

  • Conduct comprehensive assessments of individuals' needs, risks, and strengths.
  • Develop and implement personalised care plans, ensuring they are regularly reviewed and updated.
  • Provide information, advice, and support to service users and their families.
  • Collaborate with multi-disciplinary teams to ensure a holistic approach to care.
  • Advocate for the rights and needs of service users.
  • Maintain accurate and up-to-date case records and reports.
  • Ensure compliance with relevant legislation, policies, and procedures.

Qualifications and Experience:

  • A degree in Social Work or equivalent qualification.
  • Registration with Social Work England.
  • Experience working with older people and/or individuals with physical disabilities.
  • Strong assessment and care planning skills.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively within a multi-disciplinary team.
  • Knowledge of relevant legislation and best practices in social work.
