Responsibilities include:
- Conduct detailed assessments of children's needs within the Connected Carers Team, in accordance with the Children Act 1989 and relevant fostering legislation.
- Develop and implement personalised support plans aimed at promoting the safety, development, and well-being of children in connected care placements.
- Safeguard children in kinship and connected care arrangements, ensuring their protection and well-being through suitable interventions.
- Work closely with multidisciplinary teams, families, and community services to provide comprehensive, child-centered support tailored to connected carers.
- Advocate for the rights of children in connected care, ensuring they have access to necessary resources, services, and support networks.
- Regularly review and adjust support plans to address the evolving needs and circumstances of children in connected care.
- Ensure compliance with statutory duties and uphold best practices in safeguarding and connected care support.
Requirements:
- 3 years experience post qualification
- Action safeguarding procedures
