Barnet Council is seeking a dedicated Social Worker to join the Adults Early Intervention Team. Our strengths-based approach supports residents in maintaining their independence and well-being, ensuring they receive the care they need in their community.

About the Role
  • Facilitate timely, safe hospital discharges and support individuals in their transition back to the community.

  • Conduct complex assessments and safeguarding enquiries, ensuring proportionate and effective support plans.

  • Work collaboratively with adults, families, carers, health professionals, and partner organisations.

  • Undertake Mental Capacity Act assessments in line with statutory guidance.

About You
  • Qualified Social Worker, registered with Social Work England.

  • Experience working with vulnerable adults in a social work setting.

  • Strong assessment and report-writing skills.
