Barnet Council is seeking a dedicated Social Worker to join the Adults Early Intervention Team. Our strengths-based approach supports residents in maintaining their independence and well-being, ensuring they receive the care they need in their community.
About the Role
Facilitate timely, safe hospital discharges and support individuals in their transition back to the community.
Conduct complex assessments and safeguarding enquiries, ensuring proportionate and effective support plans.
Work collaboratively with adults, families, carers, health professionals, and partner organisations.
Undertake Mental Capacity Act assessments in line with statutory guidance.

About You
Qualified Social Worker, registered with Social Work England.
Experience working with vulnerable adults in a social work setting.
Strong assessment and report-writing skills.
A commitment to person-centred and strengths-based practice.
