Providing support to patients in their place or residence or spot purchase intermediate care bed in residential care homes. ESSENTIAL SKILLS Manage a clinical caseload with autonomy.Demonstrate knowledge, experience & relevant clinical skills for patients with complex/long term conditions, falls, orthopaedics & general mobility problems.Specialist level of physiotherapy clinical reasoning, moving and handling, treatment skills and techniques related to the service area.Undertake comprehensive assessment, provide therapeutic diagnosis using investigative, advanced analytical skills, specialist listening, physical & observational skills, interpret clinical findings/documentation & implement agreed treatment plans.Monitor & evaluate treatment to measure progress/outcomes to ensure effectiveness of interventions & service delivery.Develop programmes of care to support discharge.Have high level communication & excellent interpersonal skills.PREFERRED SKILLS Previous experience within Intermediate Care, supporting older adults or community service experience. QUALIFICATIONS REQUIRED BSc in Physiotherapy level 2:2 or above or equivalent.
