Job Overview:
We are seeking a highly skilled and compassionate Primary Care Paramedicto join our team. In this role, you will be responsible for leading the assessment, diagnosis, planning, implementation, and evaluation of care for patients presenting with a range of clinical needs, including undifferentiated diagnoses and complex conditions. You will work within a multidisciplinary team, providing high-quality, patient-centered care and supporting the management of both acute and chronic conditions.#

Key Responsibilities:

- Care Monitoring & Evaluation:Regularly monitor, review, and evaluate care interventions to ensure the highest quality of care is maintained.

- Assessment & Diagnosis:Lead the assessment and clinical examination of patients from both physiological and psychological perspectives, planning and implementing care accordingly.
- Care Planning & Treatment:Proactively assess, diagnose, and plan interventions for patients with complex needs, ensuring evidence-based treatments are provided.
- Long-term Condition Management:Identify patients at risk of developing long-term conditions, providing appropriate diagnoses and proactive management plans.
- Acute & Chronic Care Management:Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug treatments into a comprehensive management plan.
- Medication Management:Prescribe, review, and monitor medication for therapeutic effectiveness, in line with evidence-based practice and national/local protocols, ensuring it is within scope of practice.
- Patient Support:Work closely with patients to ensure compliance with prescribed treatments, offering support to encourage adherence to care plans.

Requirements

Qualifications:HCPC registered Paramedic with advanced clinical training or relevant paramedic degree.
- Experience: Proven experience in assessing, diagnosing, and managing patients in a clinical setting, including both acute and chronic conditions.
- Skills:Excellent clinical decision-making, patient management, and communication skills.
- Ability to Work Independently:Capability to work autonomously while also collaborating with multidisciplinary teams.
