Job OverviewAs a Case Worker, you will play a vital role in supporting individuals and families in navigating complex social systems. Your primary responsibility will be to assess client needs, develop tailored support plans, and facilitate access to essential services. You will work collaboratively with clients, community organisations, and other professionals to ensure that individuals receive the assistance they require to improve their circumstances.

Duties

  • Develop and implement Care Act Assessments & Support Planning.
  • Mental Capacity assessments.
  • Facilitate access and funding for services required.
  • Maintain accurate and confidential case records and documentation.
  • Monitor client progress and adjust support plans as necessary.
  • Advocate for clients within various systems, including social services and healthcare.
  • Collaborate with multidisciplinary teams to enhance service delivery.
