Key Responsibilities:
  • Manage referrals, assess client needs, and tailor support to sustain housing and achieve positive life changes.
  • Progress, track, and report on referrals and service user outcomes.
  • Develop, implement, and monitor client support plans and risk assessments.
  • Provide clients with practical advice and signpost them to relevant services.
  • Facilitate smooth transitions into supported housing, ensuring clients understand their rights and responsibilities.
  • Maintain accurate records, ensuring compliance with service standards and confidentiality.
What We’re Looking For:
  • Experience working with individuals in the criminal justice system or vulnerable groups.
  • Strong organizational skills to manage multiple referrals and client cases.
  • Excellent communication and interpersonal skills to engage with clients and agencies effectively.
  • Knowledge of housing solutions, risk assessments, and support planning is an advantage.
