Key Responsibilities:- Manage referrals, assess client needs, and tailor support to sustain housing and achieve positive life changes.
- Progress, track, and report on referrals and service user outcomes.
- Develop, implement, and monitor client support plans and risk assessments.
- Provide clients with practical advice and signpost them to relevant services.
- Facilitate smooth transitions into supported housing, ensuring clients understand their rights and responsibilities.
- Maintain accurate records, ensuring compliance with service standards and confidentiality.
- Experience working with individuals in the criminal justice system or vulnerable groups.
- Strong organizational skills to manage multiple referrals and client cases.
- Excellent communication and interpersonal skills to engage with clients and agencies effectively.
- Knowledge of housing solutions, risk assessments, and support planning is an advantage.
