A fantastic opportunity has arisen for a dedicated and experienced Band 6 Occupational Therapist to join a dynamic team in Surrey. The successful candidate will be working in an acute setting and will play a key role in delivering high-quality care to patients with a variety of needs, particularly focusing on discharge planning and rehabilitation.

Location: Surrey
Contract Type: Full-time (open to part-time applicants)
Salary: £25-26 per hour
Start Date: ASAP

Key Responsibilities:

  • Conduct initial and functional assessments of patients to determine their rehabilitation and care needs.
  • Plan and implement discharge plans, ensuring a seamless transition from hospital to home.
  • Provide assessments and prescriptions for equipment to support patient independence and safety post-discharge.
  • Work closely with community teams to ensure appropriate follow-up care is in place for patients upon discharge.
  • Collaborate with multidisciplinary teams to develop comprehensive care packages tailored to each patient's specific needs.
  • Maintain accurate patient records and ensure compliance with all relevant policies and procedures.
  • Support patients and families through the rehabilitation and discharge process, providing education and resources for continued care.

Key Requirements:

  • HCPC registered Occupational Therapist.
  • Proven experience working in an acute setting, with strong skills in initial assessments, functional assessments, discharge planning, and equipment prescription.
  • Previous experience in oncology is highly desirable, but not essential.
  • Experience liaising with community teams to ensure smooth patient discharge and post-hospital care.
  • Strong communication skills, with the ability to collaborate effectively with patients, families, and multidisciplinary teams.
  • Ability to work autonomously, managing a varied and often complex caseload.
  • Compassionate and patient-centred approach, ensuring the well-being of all patients throughout their care journey.
