Are you passionate about delivering high-quality care while guiding and supporting a dedicated team? We are looking for a motivated and compassionate Care Coordinator to join our team.

Key Responsibilities:

  • Supervising Staff: Conduct regular staff supervisions and lead productive team meetings.
  • Liaison Work: Act as a key point of contact between our team, families, advocates, and other professionals.
  • Care Planning: Regularly update care plans and risk assessments to ensure the highest standards of care.
  • Medication Management: Oversee the management of medication, including completing Medication Competency Assessments.
  • Reporting: Compile monthly reports on client progress and service outcomes.
  • On-Call Duties: Be available for on-call duties 3 days per month at £50 per day.

Who We're Looking For:

If you have experience in care coordination, strong communication skills, and a commitment to excellence in care, we’d love to hear from you. This role is ideal for someone who is organised, empathetic, and able to manage multiple responsibilities effectively.

On-Call Rate: £150 per on call (Friday 5pm - Monday 9am)Hours: 37.5 hours delivering community-based support within supported living services or individual’s own home or local community and carrying out administrative duties from the head office.
