Bexley Council are looking for a Care Broker to join their Adult Social Care Brokerage and Commissioning Team.

Responsibilities:

  • Support adults with physical, mental or learning disabilities to access care services either by way of a commissioned service on their behalf or to support them in managing their own care package through the provision of a personalized budget. You will over see the smooth delivery of this service
  • Take responsibility as a Care Broker across all 3 LCN's to allow good communication and a seamless service
  • Arrange homecare services and assist with placements for people who have an assessed need
  • Be part of a team who supports people who wish to take a direct payment and manage their own services
  • Ensure that discharges from hospital and the provision of community services are made with accredited providers in a timely manager
