Fernbank Healthcare have partnered with a PCN on a strategic appointment of a Advanced Nurse Practitioner to work a temporary contract until March 2025 with the possibility of an extension.
Duties:
- Assess, diagnose, plan, implement and evaluate treatment interventions in patients presenting with minor illnesses and long-term conditions (as appropriate)
- Diagnose and manage acute and some chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.
- Prescribe medications for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national practice protocols within scope of practice.
- Patient support for compliance and adherence to prescribed treatments.
- Prioritize health problems and intervene appropriately to assist patients in complex urgent or emergency situations including initiation of effective emergency care.
- Support and empower patients to adopt health promotion strategies for enhancing healthy lifestyles.
Experience of working within a GP practice is ideal.
