Band 6 CHC Assessor – £40 per hour – Birmingham (Hybrid Working)

Location: Birmingham (Hybrid – must attend on-site visits)
Start Date: ASAP
Contract Length: 3 months (potential for ongoing contract)
Working Hours: Monday – Friday, 09:00 AM – 17:00 PM
Pay Rate: £40 per hour

We are seeking an experienced Band 6 CHC Assessor to join a dedicated team in Birmingham on a 3-month contract, with the potential for extension. This role offers flexible hybrid working, but the successful candidate must be available to conduct on-site patient assessments as required.

Key Responsibilities:
  • Conducting comprehensive Continuing Healthcare (CHC) assessments in line with national frameworks
  • Reviewing care packages and making recommendations for eligibility and funding
  • Liaising with multidisciplinary teams, patients, and families to ensure the best possible care
  • Ensuring compliance with NHS Continuing Healthcare guidelines and policies
  • Managing caseloads efficiently and maintaining accurate documentation
Requirements:
  • Registered Nurse, Social Worker, or Allied Health Professional with relevant CHC experience
  • Previous experience conducting CHC assessments and DST (Decision Support Tool) reviews
  • Strong understanding of NHS CHC framework and legislation
  • Excellent communication, analytical, and report-writing skills
  • Ability to work independently while collaborating with multidisciplinary teams
