Job Summary:
A well-established, patient-centered medical practice in Birmingham is looking for a dedicated and experienced Practice Nurse to join their dynamic healthcare team. The successful candidate will play a crucial role in delivering high-quality, compassionate care to a diverse patient population.
Key Responsibilities:
Deliver comprehensive nursing care, including managing chronic conditions, wound care, immunizations, and health screenings.
Conduct patient assessments, create personalized care plans, and ensure the delivery of patient-focused care.
Work collaboratively with GPs, healthcare assistants, and other multidisciplinary team members to ensure seamless and coordinated care.
Provide patient education and promote health and wellness, encouraging individuals to take control of their own health.
Maintain accurate and up-to-date patient records, adhering to all relevant guidelines and protocols.
Qualifications and Experience:
Registered Nurse (RGN) with an active NMC pin.
Previous experience in primary care or community healthcare is highly desirable.
Proficient in managing chronic diseases such as diabetes, asthma, and COPD.
Excellent communication and teamwork skills.
Competent in using clinical systems and electronic patient records.
