Job Overview:

In this role, you will provide evidence-based interventions, manage a caseload, and undertake assessments and reviews as part of the Care Programme Approach. Acting as a care coordinator, you'll communicate effectively with all involved in the care of individuals in custody up to their release. You'll collaborate with individuals, carers, and professionals to deliver a dynamic, integrated mental health and substance misuse service, supporting rehabilitation and transitioning care to other providers as needed. Your work will contribute to helping service users live fulfilling lives within the community.

Responsibilities:

  • Conduct comprehensive mental health assessments and develop individualized care plans for prisoners.
  • Provide therapeutic interventions, including counseling and group therapy, to support mental health recovery.
  • Administer and monitor medications in line with clinical guidelines, ensuring safety and effectiveness.
  • Collaborate with prison staff to create a supportive and secure environment for patients.
  • Maintain accurate patient records, ensuring confidentiality and adherence to healthcare and legal standards.

Experience/Skills:

  • Experience of working within a multi-disciplinary team structure
  • Essential Prison clearance
  • Band 6 with prison experience mandatory
  • Experience of assessing, planning, implementing and reviewing care needs
