Purpose of the Job:

  • Deliver outstanding services to care leavers with energy and enthusiasm.
  • Provide support and interventions to young people aged 16-25, both in and leaving care.
  • Develop positive relationships and offer practical and emotional support based on assessments and pathway plans.

Main Areas of Responsibility:

  • Build relationships and communicate effectively to help young people reach their full potential.
  • Support independent living, accommodation options, education, training, employment, health, and social networks.
  • Participate in service development and ensure compliance with legislation and guidance.
  • Collaborate with other agencies to engage young people in opportunities and support.
  • Promote good practice models and adhere to the leaving care offer.
  • Ensure high standards of case recording and administrative records.
  • Promote and develop the Borough’s priorities and participate in training.

Knowledge, Skills & Abilities:

  • NVQ level 4 in child care/social care or equivalent qualification.
  • Understanding of leaving care and children legislation, child protection, and safeguarding.
  • Experience working with young people up to 25 years, including those vulnerable to exploitation.
  • Ability to communicate effectively, support resilience, work flexibly, and maintain confidentiality.
  • Commitment to equal opportunities, user involvement, and continuous professional development.

Other Requirements:

  • Legally entitled to work in the UK.
  • Willingness to work outside normal office hours.
  • Enhanced DBS check required.
