Purpose of the Job:
- Deliver outstanding services to care leavers with energy and enthusiasm.
- Provide support and interventions to young people aged 16-25, both in and leaving care.
- Develop positive relationships and offer practical and emotional support based on assessments and pathway plans.
Main Areas of Responsibility:
- Build relationships and communicate effectively to help young people reach their full potential.
- Support independent living, accommodation options, education, training, employment, health, and social networks.
- Participate in service development and ensure compliance with legislation and guidance.
- Collaborate with other agencies to engage young people in opportunities and support.
- Promote good practice models and adhere to the leaving care offer.
- Ensure high standards of case recording and administrative records.
- Promote and develop the Borough’s priorities and participate in training.
Knowledge, Skills & Abilities:
- NVQ level 4 in child care/social care or equivalent qualification.
- Understanding of leaving care and children legislation, child protection, and safeguarding.
- Experience working with young people up to 25 years, including those vulnerable to exploitation.
- Ability to communicate effectively, support resilience, work flexibly, and maintain confidentiality.
- Commitment to equal opportunities, user involvement, and continuous professional development.
Other Requirements:
- Legally entitled to work in the UK.
- Willingness to work outside normal office hours.
- Enhanced DBS check required.
