We are looking to recruit an experienced Clinical Pharmacist / Independent Prescriber to join a multi-disciplinary team based in Blackpool.
Responsibilities:
- Record medication review plans
- Provide specialist expertise in the use of medicines
- Be responsible for the care management of patients with chronic diseases
- Identify and carry out target medication reviews for high risk patients

Requirements:
- Be GPhC registered
- Hold a degree in Pharmacy with post graduate experience in pharmacy
- Independent Prescriber, qualified from approved training pathway or equivalent
