This worker will support Discharge to Assess pathways and will need to be proactive and work to a high degree of autonomy while being part of a Team. You will need to be flexible in your approach and be able to work within a Multi-disciplinary setting. You will need a good understanding of all Social Care legislation and understand the Continuing Health Care process with respect to checklist consideration, completion and DST meetings. The successful applicant will be skilled in undertaking Safeguarding work and any crisis work linked to the role. Work alongside all relevant step down and step up services such as Rapid Response to avoid any admissions/readmissions back into a hospital setting. Support the onward commissioning of services following the Discharge to Assesses Process paying attention to the needs of others involved in care planning such as family, carers friends etc. The successful applicant will also be part of a wider health and social care team.Contact me on or for more information.''
