Role Summary:We are seeking an experienced Social Worker or Mental Health Nurse to join the Fylde and Wyre Child and Adolescent Mental Health Services (CAMHS) team. This role is focused on providing direct, face-to-face support for young people, managing assessments, care plans, and hospital discharge plans. You’ll collaborate with multidisciplinary teams, manage caseloads, and ensure all interventions meet the organization’s care thresholds.

Key Responsibilities:

  • Conduct independent assessments, care planning, and hospital discharge planning.
  • Adapt and manage risk plans according to changing needs.
  • Provide triage and duty support, prioritizing caseloads effectively.
  • Signpost to appropriate services and make safeguarding referrals as necessary.
  • Work directly with young people with diverse mental health and wellbeing needs.
  • Support the team collaboratively, ensuring effective discharge planning.
  • Work within established systems to maintain high standards of care and compliance.

Essential Skills and Experience:

  • Previous experience in CAMHS or child protection work, particularly with young people.
  • Proficiency in independent assessments, risk management, and care planning.
  • Ability to support the team dynamically, adapting to changes in risk and workload.
  • Familiarity with safeguarding practices and multi-agency collaboration.

Requirements:

  • Face-to-face working only; must be available for in-person support at Whitegate Drive Health Centre.
