This service covers Admission Avoidance; Intermediate Care and Discharge to Assess bed-based units.About You: - We are looking to for an enthusiastic and motivated individual who has a passion for working with adults, understanding their needs, aspirations and supporting them to enhance their quality of life and promote independence.As a Social Worker in IMC, you will work in one of the designated services and undertake assessments/reviews of need under the Care Act 2014, assist with support planning as necessary and also link people into universal and voluntary services.
