Social Worker - Adults

About the Role:
An exciting opportunity has arisen for a passionate and experienced Adult Social Worker to join our Learning Disability team at Bournemouth Christchurch and Poole Council. This is a care management role that requires a high level of autonomy and the ability to work effectively with people with complex needs.
- This is not a remote working role.
- The successful candidate will be required to carry out face-to-face visits in the local area and be office-based at least 2 out of 5 days.
- The remaining days can be worked from home.
Key Responsibilities:
- Carrying out Care Act assessments and capacity assessments
- Devising and implementing person-centered care and support plans
- Undertaking safeguarding enquiries
- Working closely with families and carers to ensure their needs are met
- Representing the Council at multi-agency meetings
- Providing advice and guidance to other professionals
Essential Requirements:
- Degree or Diploma in Social Work
- Current registration with Social Work England
- Experience of working with adults with learning disabilities
- Knowledge of the Care Act 2014 and Mental Capacity Act 2005
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
