Qualified Social Worker - Hospital Discharge TeamWe are looking to recruit a Qualified Social Worker - Hospital Discharge team with the Social Care industry in Bournemouth Christchurch & Poole Council. The Qualified Social Worker - Hospital Discharge team vacancy in Hampshire will be for an initial 3-6 months with potential for extension subject to internal circumstance and your performance..

Responsibilities include:

  • To assist the appropriate manager in discharging the full range of statutory duties and
  • BCP Council objectives, within the team which the post holder works,
  • under the supervision, guidance and direction of the relevant manager.
  • To manage a caseload appropriate to the primary function of the team, acting as the
  • case coordinator or 'lead professional', undertaking appropriate assessments, and
  • contributing to and coordinating multi-agency multi disciplinary teams and/or
  • conferences where appropriate.
  • To ensure that all service users including adults, families and carers are fully enabled
  • to participate in planning and decision making concerning their own lives and that
  • complaints and representations are dealt with effectively.
  • To participate in and contribute to the continuous improvement and development of the
  • adult's services, including providing oversight and support to other professionals.
  • To make sound professional and financial decisions in accordance with policies and
  • To ensure that work undertaken meets expectations as outlined in service unit and
  • BCP Council quality assurance and performance management systems.
  • To design and negotiate individual care packages, with the service user, their family or
  • Undertake direct work with individual service users, their families and/or carers as part of an agreed plan, and within this promoting health and wellbeing.

Experience preferred:

  • Post qualifying experience of working within a Qualified Social Worker - Hospital Discharge team within the UK Social Care industry.
