Children Social Worker Team: CCHDTLocation: BradfordRate: £3537 Hours Per Week - Bradford Metropolitan District Council

The Children's Complex Health or Disabilities Team (CCHDT) in Bradford provides a specialist Social Care support to disabled children and young people (0-18) with needs arising from a physical or mental impairment that are severe, profound or complex and have a permanent and substantial and long-term adverse effect on the child/young person and/or their families.

There are 4 locality CCHDT Social Work teams which cover the Bradford district, and work with children with complex health needs and/or disabilities.

The CCHDT Social Work teams work to the same statutory requirements of other Social Work teams. Each team undertakes assessments, safeguarding and care management activities. They also work with children in care.

The teams assess on-going care needs and develop specialist packages of support for these children. This includes assessing the impact of caring responsibilities on the family as a whole, including consideration of the needs of siblings. This is done through Child and Family Assessments, parent carer assessments, regular reviews and attending resource panels.

Where specialist Social Work support is not required, but there remains a need for support for the child/young person in relation to short breaks which can’t be met within Universal, Early Help or by the SEND short breaks team, a team of community resource workers within CCHDT continue to review the support packages in place.
