Job OverviewAn excellent opportunity has arisen for a Band 6 Community Occupational Therapist to join the South Staffordshire Integrated Community Occupational Therapy Team - Adult Social Care. Based in Lichfield/Burton, this temporary position is ideal for an experienced Occupational Therapist with a passion for supporting adults with physical disabilities and long-term health conditions. The role focuses on enabling individuals to maintain independence within their homes and community through targeted occupational therapy interventions.
Key Responsibilities- Provide assessments and interventions to support adults with physical disabilities and long-term health conditions.
- Develop personalized care plans to promote independence and quality of life.
- Work closely with individuals and their families to identify needs and deliver appropriate solutions.
- Collaborate with a multidisciplinary team to ensure a coordinated approach to care.
- Monitor and review progress, making necessary adjustments to interventions.
- Contribute to the team's goals by engaging in service development and clinical audits.
- HCPC Registration: Essential.
- Experience: Previous experience in social care occupational therapy or community settings is highly desirable.
- Skills: Strong communication, team-working, and problem-solving skills.
- Knowledge: Familiarity with supporting adults with physical disabilities and long-term conditions.
