- Conduct face-to-face Care Act assessments for individuals with complex needs.
- Support and coach Social Care Workers within the ABI Unit to enhance practice and performance.
- Participate in Multidisciplinary Team (MDT) meetings (mainly via Teams, with some in-person attendance).
- Provide supervision and oversight of cases within the team, ensuring timely and effective management of complex caseloads.
- Challenge health professionals appropriately where required in CHC assessments and decision-making.
- Conduct casefile audits, promote shared learning, and contribute to team development and partnership working.
- Travel to ABI units and attend meetings as required (travel expenses reimbursed after the first journey from home).
- Qualified Social Worker registered with Social Work England.
- Experience in Acquired Brain Injury (ABI) and Continuing Health Care (CHC) processes.
- Strong knowledge of the Care Act 2014 and experience in complex case management.
- Ability to provide leadership, supervision, and coaching to Social Care Workers.
