Senior Practitioner, Discharge to Assess Community Team, West MidlandsRole Overview:Pertemps is seeking a Senior Practitioner to join the Discharge to Assess Community Team (North), supporting individuals with Acquired Brain Injury (ABI). This role involves complex Care Act assessments, participation in the Continuing Health Care (CHC) process, and working within strict timescales to ensure high-quality service delivery.Key Responsibilities:
  • Conduct face-to-face Care Act assessments for individuals with complex needs.
  • Support and coach Social Care Workers within the ABI Unit to enhance practice and performance.
  • Participate in Multidisciplinary Team (MDT) meetings (mainly via Teams, with some in-person attendance).
  • Provide supervision and oversight of cases within the team, ensuring timely and effective management of complex caseloads.
  • Challenge health professionals appropriately where required in CHC assessments and decision-making.
  • Conduct casefile audits, promote shared learning, and contribute to team development and partnership working.
  • Travel to ABI units and attend meetings as required (travel expenses reimbursed after the first journey from home).
Key Requirements:
  • Qualified Social Worker registered with Social Work England.
  • Experience in Acquired Brain Injury (ABI) and Continuing Health Care (CHC) processes.
  • Strong knowledge of the Care Act 2014 and experience in complex case management.
  • Ability to provide leadership, supervision, and coaching to Social Care Workers.
