- Conduct assessments and create support plans for individuals with Learning Disabilities, Autism, and Physical Disabilities.
- Undertake Mental Capacity Assessments and make Best Interest Decisions in line with the Mental Capacity Act 2005.
- Work in partnership with multi-disciplinary teams, carers, and external agencies to deliver person-centred care.
- Carry out Safeguarding Investigations in line with adult safeguarding procedures.
- Apply knowledge of key legislation, including the Care Act 2014, Continuing Health Care, Mental Health Act, and Community DoLS/LPS.
- Support individuals with profound and complex needs to achieve the best possible outcomes.
- Qualified Social Worker registered with Social Work England.
- Experience in Adult Social Care with a focus on Learning Disabilities and Autism.
- Strong working knowledge of relevant legislation and policies.
- Ability to work collaboratively with a diverse range of service users and partner agencies.
- Excellent assessment, safeguarding, and case management skills.
- Flexible, committed, and motivated to deliver high-quality social work practice.
