Social Worker, YAT/Transitions Team, West MidlandsRole Overview:Pertemps is seeking an experienced and dedicated Social Worker to join the Young Adults & Transitions (YAT) Team. This is an exciting opportunity to be part of a newly restructured Adult Social Care team, supporting young adults with Learning Disabilities and Autism as they transition into adult services.Key Responsibilities:
  • Conduct assessments and create support plans for individuals with Learning Disabilities, Autism, and Physical Disabilities.
  • Undertake Mental Capacity Assessments and make Best Interest Decisions in line with the Mental Capacity Act 2005.
  • Work in partnership with multi-disciplinary teams, carers, and external agencies to deliver person-centred care.
  • Carry out Safeguarding Investigations in line with adult safeguarding procedures.
  • Apply knowledge of key legislation, including the Care Act 2014, Continuing Health Care, Mental Health Act, and Community DoLS/LPS.
  • Support individuals with profound and complex needs to achieve the best possible outcomes.
Key Requirements:
  • Qualified Social Worker registered with Social Work England.
  • Experience in Adult Social Care with a focus on Learning Disabilities and Autism.
  • Strong working knowledge of relevant legislation and policies.
  • Ability to work collaboratively with a diverse range of service users and partner agencies.
  • Excellent assessment, safeguarding, and case management skills.
  • Flexible, committed, and motivated to deliver high-quality social work practice.
