Your new companyThis is an exciting time to join Sandwell Metropolitan Brought Council Adult Social Care. We are currently looking to recruit an agency social worker on 37 hours into ourHospital 2 Home team.

Your new roleSandwell MBC are looking for a committed, hardworking, qualified Social Worker for a borough-wide Hospital 2 Home team.

As a qualified Social Worker, you will work in various locations and will be responsible for supporting the timely discharge of people from hospital into intermediate care settings. You will be required to work in partnership with the NHS and the ICB and will have a strong customer focus.You will be working with older people, people with physical disabilities and sensory impairments, as well as mental health issues. You will need to be motivated and flexible with high professional standards and sound knowledge and experience of current legislation, including the Care Act 2014, Safeguarding, Continuing Health Care, Mental Health Act, Mental Capacity Act 2005, and Community DOLS/LPS.

You will be working in a friendly environment, with a skilled, diverse and supportive team.

CONTRACT: 3-month rolling contractWork Location - Various from Midland Metropolitan University Hospital (MMUH) or Sandwell Hospital

Work Schedule Monday - Friday

What you'll need to succeedTo be successful, you will need to

  • Have a recognised professional Social Work qualification (MA/BA/B.Sc. in Social Work, Diploma in Social Work or CQSW)
  • Social Work England registration
  • 1 year post-qualifying experience.
  • Previous experience working within Social Work adults services
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to travel as required.
