Our client is looking for a passionate and proactive Care Co-ordinator to join their exceptional team.

In this dynamic role, you will be at the heart of our operations, expertly managing the scheduling and coordination of the companys outstanding care services in Bridport.

Your dedication will ensure that every client receives unparalleled care and attention, embodying the high standards we stand for.


Key Responsibilities:

Coordinate and manage the daily schedules of care workers to ensure timely and efficient service delivery.
Develop and maintain care plans tailored to the individual needs of clients.
Ensure compliance with health and safety regulations and company policies.
Perform risk assessments and reviews of care packages.
Act as the main point of contact for clients, their families, and healthcare professionals.
Handle any complaints or concerns promptly and professionally.
Maintain accurate records and documentation of care provided.
Support and supervise care workers, providing guidance and training as needed.
Liaise with external agencies and professionals to ensure a cohesive approach to care.
Conduct assessments of new clients and reassessments of existing clients to ensure their needs are met.

Requirements:

Previous experience in a care coordination role is essential.
NVQ Level 2/3 in Health and Social Care is preferable.
Strong understanding of CQC regulations and compliance.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Compassionate and empathetic nature with a commitment to providing high-quality care.
Proficiency in using electronic care planning systems is an advantage.
Strong organisational and multitasking skills.


Schedule:


Monday to Friday
Weekend availability

Experience:


providing care: 1 year (preferred)
supervisory: 1 year (preferred)

Licence/Certification:


Driving License (preferred)
NVQ Level 2 Health & Social Care (preferred)
