About the Role:You will be part of a skilled and supportive therapy team working across three frailty wards, including a specialist dementia ward. The integrated therapy approach within the team allows you to conduct comprehensive, generic, and holistic assessments that make a real difference to patients' lives.

Key Responsibilities:

Deliver high-quality, patient-centered care through tailored assessments and interventions.
Work collaboratively with Occupational Therapists, Physiotherapists, and Therapy Assistants to provide seamless therapy services.
Carry out holistic and innovative assessments for a range of frailty and dementia patients.
Contribute to discharge planning and support patients in achieving their functional goals.
Ensure excellent communication with patients, families, and other healthcare professionals.
