About the Role

We are seeking a dedicated and experienced Band 6 Physiotherapist to join our community-based stroke rehabilitation team. This rewarding role involves providing high-quality assessment and tailored rehabilitation for individuals recovering from a stroke, supporting them both immediately upon discharge from hospital and in their longer-term recovery journey.

You will work in patients' homes across the Bristol, North Somerset, and South Gloucestershire areas, delivering care that empowers individuals to regain independence and improve their quality of life.

Key Responsibilities
  • Conduct in-depth physiotherapy assessments of patients recovering from stroke in their homes.
  • Develop and implement personalized rehabilitation plans to meet individual needs and goals.
  • Monitor progress, adjusting treatment plans as needed to optimize outcomes.
  • Collaborate with a multidisciplinary team, including occupational therapists, speech therapists, and other healthcare professionals.
  • Provide guidance, support, and education to patients, families, and carers.
  • Maintain accurate documentation and patient records.
  • Travel between community locations as required to meet service demands.
Requirements
  • HCPC registration as a Physiotherapist.
  • Proven experience in stroke rehabilitation (essential).
  • Community-based experience (essential).
  • Strong interpersonal and communication skills.
  • Ability to work autonomously and manage a caseload effectively.
  • Full UK driving license and access to a vehicle (preferred).
