About the RoleWe are seeking a highly motivated and experienced Band 6 Occupational Therapist to join our dynamic and fast-paced community-based stroke rehabilitation team. This is an exciting opportunity to make a real difference in the lives of individuals recovering from stroke, providing expert assessment and specialist rehabilitation services.
You will work as part of a supportive and multidisciplinary team, delivering high-quality care tailored to each individual's needs. The role involves working across the Bristol, North Somerset, and South Gloucestershire regions, offering a varied and engaging caseload in community settings.
Key Responsibilities- Conduct comprehensive assessments of patients following stroke, identifying their rehabilitation needs.
- Develop and implement individualized rehabilitation plans to promote recovery and independence.
- Collaborate with other healthcare professionals to deliver coordinated and holistic care.
- Provide advice, education, and support to patients, families, and carers.
- Maintain accurate and timely patient records.
- Travel between community locations as needed to meet service requirements.
- HCPC registration as an Occupational Therapist.
- Previous experience in stroke or neuro rehabilitation (essential).
- Experience in community-based roles (essential).
- Strong communication and interpersonal skills.
- Ability to work flexibly across multiple locations.
- Full UK driving license and access to a vehicle (preferred).
