Hospital Discharge / Settle IN Worker – Homelessness  LOCATION:               Bristol   HOURS:                     37.5SALARY:                   £13.50 PAYE £14.22 UMBRELLADURATION:  2 months +START DATE:           ASAP The Settle-in workers will work intensively with service users in the period following discharge. The model is seeking to instil continuity of care and a multi-disciplinary approach focussing on the needs of the individual; Settle-in workers will work alongside GP services, community outreach nurses, social care and housing colleagues and peer supporters to ensure that housing, health & care needs of people using the service are met, minimising the potential for return to hospital or the streets Key Responsibilities & Duties 
  • Support a low to medium caseload of homeless clients
  • Using a positive risk-taking approach, ensuring health and safety policies are implemented and fully adhered to ensure all safeguarding issues are properly managed.
  • Provide pro-active and responsive support to the Service Users who are identified. This will include working with the service user and their support network, in the community on practical issues like preparing accommodation, getting food, electric etc. prior to discharge.
  • Proactively use a Recovery orientated, trauma and psychologically informed, equalities-based approach in all interactions.
  • As this is a pilot, Settle-in workers will contribute to and benefit from local and national learning.
  • Screen referrals made, signposting to alternative pathways as appropriate to meet identified needs
  • Support service users to maintain their property through compliance checks such as health & safety, and fire checks and when necessary, arrange maintenance repairs.
