Tate are working with a local authority in Bristol in their search for a SEND Assessment Coordinator.
Key Responsibilities:
- Being the lead contact for Education, Health and Care Plan (EHCP) activity for children and young people (CYP), with special educational needs and disability and their families
- Coordinate activities that are required to assess needs, review the specified needs to ensure they are being met, monitor if progress is being made and champion inclusion in the education setting
- Coordinate practitioners and services around the child, young person and their family, identifying when specialist (Social Workers, Educational Psychologists, Occupational Therapists, other professionals etc.) involvement is required, make the appropriate contacts and ensure effective liaison with the specialists
- Arranging person centred multidisciplinary and/or multiagency meetings as required
- Ensure that each need identified in the assessment is matched to an outcome in the plan
- Being responsible for drafting and finalising single plans, including those with statutory protections and agree the plan with young people and their families
Skills/experience:
- SEND experience
- Complex case experience
- Familiar with Annual Review processes for children and young people with EHCP's
