In this rewarding role, you'll play a key part in supporting individuals with everyday activities, while also leading and inspiring a small, dedicated team of support workers. As a Support Coordinator, you'll work closely with the people you support, their families, the community, and other members of their support network to ensure they receive the best care possible.

What We're Looking for:

  • Experience in health and social care, supporting individuals with a variety of needs, including those with learning disabilities, mental health challenges, physical disabilities, or autism.
  • A caring and adaptable approach to writing clear risk assessments and support plans.
  • Experience leading teams, holding team meetings, and supervising staff to ensure top-notch care.
  • Desirable: Knowledge of current health and social care legislation and best practices.
  • Car drivers
