Your responsibilities will also include:• Conducting assessments and developing tailored care plans for service users and carers, ensuring their unique needs and priorities are addressed.• Advocating choice and control, you'll empower individuals to make decisions and manage risks, ensuring services are organised in alignment with their preferences and needs.• Uphold professional standards continuously as our service evolves, you'll monitor changing needs, develop support plans, utilise resources, and ensure Social Care remains future-ready through creative use of assistive tech.You will:• Be a qualified social worker with a Social Work England registration, you will have a Social Work or relevant SWE qualification.
