Full time hours on a temporary basis
Support adult social care this winter by ensuring statutory responsibilities are met and responding to seasonal demands. Key responsibilities include:

Overseeing the mobilisation of winter funding to meet increased demand

Developing a live dashboard for winter capacity and demand monitoring

Identifying system challenges and leading transformation

Developing social care pathways to reduce acute care pressures

Managing the roll-out of redirected acute funding and enhancing best practices
-
Collaborating with senior leadership to escalate issues and lead change

Developing integrated care models to address system pressures

Mainstreaming long-term models to maintain performance
-
Providing regular reporting on impact, outcomes, and financial position
