What you will do:

  • Conduct timely, strength-based assessments for vulnerable adults under the Care Act and Mental Capacity Act.
  • Arrange and coordinate necessary care support to facilitate a safe discharge and return home, thus reducing social admissions to hospital.
  • Attend daily board meetings to discuss patient flow and plan discharges effectively.
  • Engage with younger adults with physical disabilities, sensory impairments, and older people, fostering independence and carefully managing risks.
  • Collaborate within a multidisciplinary team framework, developing strong working relationships with partner agencies.
  • Demonstrate good communication and negotiation skills while working closely with carers and health colleagues.
