Job Title: Social Work Assistant
Pay Rate: 17.12 per hour
Contract: 3 months (with potential to extend or become permanent)
Hours: Full-time, 37 hours per week (Monday to Friday, 9:00 AM - 5:00 PM)
Location: Bromsgrove, B61

We're seeking a Social Work Assistant to join a dynamic team committed to delivering person-centred support that makes a real impact.
About the Role:
As a Social Work Assistant you'll:
- Conduct assessments using national eligibility criteria and create person-centred support plans.
- Provide advice, practical support, and assistance to help service users regain independence.
- Work collaboratively with individuals, families, and external agencies to achieve positive outcomes.
- Facilitate access to services, resources, and universal support systems.
- Maintain accurate records and liaise with other professionals to deliver effective care.
What We're Looking For:
- Experience: Significant experience in health or social care, particularly with individuals with learning disabilities, long-term conditions, or dementia.
- Full UK Driving Licence with unrestricted use of your own vehicle
- Qualifications: NVQ/Diploma Level 3 in Health and Social Care (or equivalent).
- Knowledge: A solid understanding of the Care Act 2014, Mental Capacity Act 2005, and person-centred support planning.
- Skills: Strong communication, organisational, and IT skills, with the ability to manage your workload effectively.
