Our client is a leading complex care provider dedicated to supporting clients to live independently at home and within their communities. They offer a supportive and dynamic work environment with a focus on delivering high-quality care.
As a Community Nurse Case Manager, you will:
- Conduct initial assessments in the community.
- Train and assess workers' competencies and allocate support workers to patients.
- Collaborate with the wider multidisciplinary team (MDT).
- Attend meetings and make referrals.
- Hold team meetings.
- Manage a team of non-clinical case managers.
The Community Nurse Case Manager role comes with a comprehensive package, including:
- Annual salary of 43,000.
- 3,000 car allowance.
- Mileage expenses.
- Full-time hours: 37.5 hours per week, Monday to Friday.
- Be an RGN or RSCN Qualified Nurse with excellent clinical skills in complex care, including peg feeds, ventilators, and tracheostomy management.
- Have experience in tracheostomy and complex care.
- Be a car driver, flexible to travel around South Yorkshire (Sheffield, Rotherham)
- Have experience caring for both adults and children.
